Anaesthesia was practiced and developed first in Dhaka Medical College & Hospital and then spread to other parts of Bangladesh. At the establishment of the hospital in 1946, there was no recognized trained anaesthesiologist, in 1948 two self trained and in 1951, Dr. K.A.S.M.A. Quader was the first appointed postgraduate anaesthetist. He was In-charge of the Department of Anaesthesia of the Dhaka Medical College & Hospital. Now, the Department of Anaesthesia, Analgesia, Palliative & Intensive Care Medicine of Dhaka Medical College is one of the most comprehensive and inter-disciplinary center. It has a diverse activities on education, research and treatment.

Academic side
Undergraduate and postgraduate education has always received a great deal of attention at the Department. At present there are 5 courses: (open for foreign students)

1. M.D- Anaesthesiology- 5 years duration with thesis.
2. M.D- Critical Care Medicine – 5 years duration with thesis.
3. FCPS in Anaesthesiology- 4 years duration (run by BCPS)
4. Diploma in Anaesthesiology- 2 years duration.
5. MCPS in Anaesthesiology- 1 years duration (run by BCPS)
There are 1 Professor, 3 Associate Professors, 3 Assistant Professors, 2 Senior Consultants and 4 Junior Consultants in the department. Conferences, seminars, workshops, scientific meetings, joint meeting with other departments are organized by the department regularly. It has a class room, a conference room and a departmental library.

Research Programs
The department always emphasizes on research. Some of research programs, ongoing and completed recently are mentioned below:
1. Transversus abdominis plane blocks as part of a multimodal analgesia to reduce opioid consumption for total abdominal hysterectomies.
2. Effects of dexamethasone vs clonidine with bupivacaine in transversus abdominis plane block patient undergoing lower segment caesarean section.
3. Role of noninvasive ventilation in the post-operative period after emergency gut surgery.
4. Effectiveness of different doses of dexmedetomidine in obtunding haemodynamic responses to laryngoscopy in controlled hypertensive patients.
5. Prevention of post extubation sore throat by inflating the cuff with distill water instead of air.
6. Postoperative hyponatremia in elderly patients undergoing major head and neck surgery.
7. Effects of verapamil adjuvant with local anaesthetic mixtures in supraclavicular brachial plexus block.
8. Effectiveness of oral clonidine and oral gabapentin in attenuation of haemodynamic stress response to laparoscopic cholecystectomy- a placebo control trial.
9. Efficacy of pregabalin in attenuation of laryngoscopy and intubation reflex- a comparison with gabapentin.
10. Efficacy of dexamethasone adjuvant in brachial plexus block- a comparison with fentanyl adjuvant.
11. Gabapentin and prevention of post-operative nausea and vomiting.
12. Efficacy of prophylactic different doses of use of intravenous ondansetron in the prevention of spinal anaesthesia induced hypotension and bradycardia in elective caesarean section.
13. Efficacy of noradrenaline and ephedrine for the treatment of intra-operative hypotension in elective caesarean section following subarachnoid block- a comparative study.
14. Dexmedetomidine for sedation during total abdominal hysterectomy under spinal anaesthesia.
15. Effect of bupivacaine and bupivacaine with dexamethasone infiltration in surgical incision port for post-operative pain management of laparoscopic cholecystectomy patient.
16. Prevention of post extubation sore throat by inflating the cuff with distill water instead of air.
17. Use of dexmedetomidine for sedation and analgesia of mechanically ventilated patients.
18. Role of lung USG for diagnosis of ARDS.
19. Effectiveness of end tidal CO2 over arterial CO2 in mechanically ventilated patient.

Treatment Facilities
Anaesthesia
The department undertakes over 24,000 anaesthetic (general and regional) procedures every year. It provides pre-operative anesthesia consultation, peri-operative care to patients including recovery units for patient monitoring after surgery. Anaesthesia is given in over 28 operation theatres in general surgery, neuro-surgery, casualty, orthopedic, urology, ENT, gynaecology, obstetric, paediatric, thoracic, eye and maxillofacial surgery and in ancillary services (endoscopy, interventional and endovascular cardiology and neurology, interventional radiology, radiotherapy).

Intensive Care Unit
Intensive Care Unit was started in Dhaka Medical College & Hospital in 1985 with six beds only. Now it hosts 32 beds, all have ventilator support.This multi-disciplinary unit provides medical and surgical care to critically-ill, severely-injured (polytrauma) and patients who have undergone major surgery requiring monitoring at close intervals, possibly combined with life-support methods such as artificial respiration, hemodynamic support or dialysis. Each year, over 1000 patients are treated within this unit.

High Dependency Unit
Under the direction of Intensive Care Unit, its High Dependency Unit, with 20 beds, provides care to patients in need of ongoing surveillance of vital signs or whose condition is too unstable for them to return to a care unit. It is an intermediate step between intensive care and a traditional patient care unit. The unit treats more than 250 patients per year.

Palliative Care Unit
Terminally ill and incurable patients take palliative care from this wing consisting 12 beds. Every year, approximately 350 patients take care from this unit.

Pain Management
Outpatient treatment of patients with chronic pain; multimodal inpatient pain management; postoperative pain management with 24/7 care; consultancy for pain care of hospitalized patients are provided by this unit.

 

Profile of Prof. Dr. Md. Mozaffer Hossain

Professor Md. Mozaffer Hossain, son of A Razzak Miah and Sakhina Begum, was born on 21st December 1963 at the village of Jariper Dangi, Chor Bishnopur under Sadarpur upazila of Faridpur. He has been working as the Professor and Head of the Department of Anaesthesia, Analgesia, Palliative & Intensive Care Medicine of Dhaka Medical College & Hospital from 5th November 2016.

He passed SSC in 1979 from Charbhadrason High School, Faridpur and HSC in 1981 from Charbhadrason College, Faridpur. He passed MBBS from Mymensingh Medical College in 1988. He obtained Diploma in Anaesthesiology in 1999 from Bangabandhu Sheikh Mujib Medical University and FCPS (Anaesthesiology) in 2003 from the College of Physicians and Surgeons, Bangladesh.

Professor Md. Mozaffer Hossain has been working as a teacher, examiner, researcher, guide and co-guide for several years. He has several publications as a principal author and co-author and presented papers several times in both the country and abroad.
